I am sorry to hear about what happened to you. Under the ADA, there is no requirement that you submit a written request for a reasonable accommodation. However, it is strongly recommended that you do put this in writing. The first step is to see if your HR department has an ADA request form for you to fill out. If they do not (and they may not) then I would send an email to someone in HR and let them know the accommodations you are requesting.
