Customer: Hi! My husband applied for fmla in ct and passed away before he received benefits- I was told he would get his benefits on a card but we never received the card. When I called I was told they can’t talk to me because he didn’t have a 3rd party authorization- I was told by the card company that I need a court evidence of authority when I asked what that was they said contact a lawyer.
JA: What kind of workplace is this (private sector, public sector, etc.)? How many employees?
Customer: It’s not a workplace it’s thru the state
JA: Have you worked there at least 12 months? And at least 1,250 hours in the 12 months before the leave?
Customer: yes he did
JA: Is there anything else the Lawyer should know before I connect you? Rest assured that they'll be able to help you.
Customer: no

Hi, my name is ***** ***** welcome to Just Answer.

You have to provide proof of death to the provider as well as evidence of your marriage. Then they'll speak with you. The funeral home can provide you with a copy of the death certificate.

Customer
I have the death certificate they said this is in addition to that
Customer
There was no power of attorney

A power of attorney would have expired upon his passing. All you need is the death certificate and proof of your marriage.

Customer
Thank you
