I am on FMLA and believe my company has violated the FMLA. 1. I never was provided a certificate showing I was approved

Customer: I am on FMLA and believe my company has violated the FMLA. 1. I never was provided a certificate showing I was approved for FMLA. 2. They rescinded an agreement to allow me to relocate closer to my family due to my medical condition without any notification or cause. 3. During my FMLA the department manager insisted on a bitch session where he scolded me. What are my options? They are obviously setting up the stage to terminate me once I come back to work.
JA: What kind of workplace is this (private sector, public sector, etc.)? How many employees?
Customer: Private sector. About 200 employees. About 125 work in the home office.
JA: Have you worked there at least 12 months? And at least 1,250 hours in the 12 months before the leave?
Customer: yes on both
JA: Is there anything else the Lawyer should know before I connect you? Rest assured that they'll be able to help you.
Customer: The benefits manager and I had a long phone conversation a few weeks back on a Friday and the following Monday she was terminated. No one there has a clue about how the FMLA works. I am on STD and have an appointment with my doctor on Thursday hoping to be released to return to work. I also have a pending work comp claim that has not been determined. I see the WC doctor today.

You must for that matter attach a copy of the medical report and send a written complaint to the EEOC and also to the department of labor if the matter doesn't get settled there. But first of all, send legal notice to the HR of the company if the company terminates you.

Customer
Do I need to hire a local attorney to draft the documents to send to the department of labor and EEOC?

You may draft them by yourself.
