Your benefits have been stopped probably because your employer is no longer treating your lay off as temporary.
The extension for CERB was just announced and details of how to apply will probably be released soon. If you applied for CERB through your CRA account, details about getting the extension will be on that website. If you applied for CERB through Service Canada, you will be getting details on how to apply through your My Service Canada website and account.
If you are not able to return to work for health reasons and have been advised by the doctor to be off work a bit longer, I would suggest that you get a medical letter from your doctor verifying his opinion. You should give this letter to your employer to ask that you be allowed to return to work later on. As you are not unionized, there is no guarantee of right of recall if you request to come back to work at a later date.
You will also need this medical letter in the event Service Canada asks for confirmation of your reasons for requesting extended CERB which they will do at a later date.
As to why you received only$ 500, can you tell me how many CERB payments you havereceived and how much each payment was ?

In that case, you received the full amount for CERB. You now have to await instructions on how to apply for the extended CERB and meanwhile, get a doctor's letter for your employer and for Service Canada.
